Community Health Needs Assessment
Community HealthCare System extends its resources beyond the clinic and hospital walls by developing partnerships with other local organizations to improve the health of the communities it serves.
In 2012, Community HealthCare System, alongside the Pottawatomie County Health Department, conducted a county wide Community Health Needs Assessment (CHNA). The CHNA was undertaken to identify priority healthcare, wellness and prevention issues which are best addressed by health providers working collaboratively. We are fortunate to have community partners who are equally committed to this valuable work which is aligned with our core mission To enrich the health and lives of the people we serve.
CHCS served as a primary partner in the Pottawatomie County CHNA and supports/participates as needed in the community health needs assessments for each county we serve. The determination of community health needs takes into account community demographics, socio-economic factors, and health service utilization trends. In 2021, we worked with our community partners to reassess community health and update our plans. This effort was even more important as we worked to help our communities fight the COVID-19 pandemic.
The assessment and formal report is in alignment with the federal IRS community benefit reporting requirements that affect all State-licensed 501(C) (3) hospitals. The priorities identified in the assessment have been accepted by the Community HealthCare System Board of Directors.
Community HealthCare System will utilize the most recent assessment, adopted in 2022, as a guide over the next three years for providing community health support for the communities we serve. Since 1955, Community HealthCare System has anticipated and adapted to meet local community healthcare needs. The 2022 CHNA is more evidence of that ongoing commitment.
