Community HealthCare System’s home health care department has been named a Top Agency of the HomeCare Elite for the 12th year straight.
The ranking is a recognition of the top-performing home health agencies in the United States and was developed by ABILITY® Network, a leading information technology company helping providers and payers simplify the administrative and clinical complexities of healthcare. It is sponsored by DecisionHealth, part of the H3.Group. Out of 8,818 agencies considered, 2,207 are recognized on the 2019 HomeCare Elite winners list overall.

Roz Lewis, director of ancillary services at Community HealthCare System, or CHCS, said the staff’s dedication to delivering personal care and going the extra mile for patients has helped achieve the HomeCare Elite recognition for so many years. "The point of home health care is to keep people out of the hospital or emergency room. We tell patients to call our nurses if they need something and it’s not an emergency, and they know we are there for them," Lewis said.
Lewis said she often hears stories of nurses’ extraordinary efforts to help patients. “You never know what difference you are making, even when it seems like you are doing an everyday thing,” Lewis said. Community HomeHealth is also celebrating a zero-deficiency-survey, meaning an outside state surveyor found nothing to correct. “The surveyor was very impressed from the start and could tell we had great relationships with patients even in difficult circumstances,” Lewis said.
Community HomeHealth provides service in Pottawatomie, Nemaha, Jackson, Marshall, and Wabaunsee counties.
HomeCare Elite agencies are determined by an analysis of performance measures in quality outcomes, best practices implementation, patient experience (HHCAHPS), quality improvement and consistency, and financial health. In order to be considered, an agency must be Medicare-certified and have data for at least three outcomes in Home Health Compare.
